Each of the following forms must be completed and filed on time by a candidate for partisan office in order for the candidate’s name to be placed on the ballot at the August 11, 2026 Primary (if necessary) and the November 3, 2026 General Election. The filing officer is the County Clerk. Candidates should contact the Lisa Schmitt, Buffalo County Clerk at 608-685-6206 for further information and to obtain the necessary forms. Forms are also available on the Wisconsin Elections Commission website.Â
- Form ELIS-3 Ballot Access Checklist
- Form EL-162 Declaration of Candidacy
- Form EL-168 Nomination Papers (You cannot circulate papers and collect signatures before April 15, 2026)
- Form CF-I Campaign Finance Registration Statement. If you plan to spend or accept any money you must have a completed Campaign Registration CF-I on file prior to the fund activity.
All paperwork must be filed in the County Clerk’s office no later than 5:00 PM on Monday, June 1, 2026.
If a current candidate does not plan to run again, a non-candidacy form needs to be completed. The deadline for incumbents to file this form is May 22, 2026.
